How to Give Feedback

If you would like to give us feedback, whether a compliment, concern or complaint, please speak to the service directly involved or contact the NHS Lothian Patient Experience Team.

Where possible, it would be helpful if you can provide us with the following information:

  • Are you the patient (yes or no).  If no, please give your name and relationship to the patient
  • Patient’s name
  • Patient’s postal address including post code
  • Patient’s date of birth
  • Your name and address, if complaining on behalf of someone else.
  • Your email address, if you have one
  • Telephone number
  • Description of the feedback that you are providing
  • Location where this occurred
  • Date when this occurred

If you are not the patient and are providing information on or behalf of someone else, we will need to obtain their consent so that we can act on this information. For further details please see our FAQ section.

Complaints

When making a complaint about your experience or on behalf of someone else, it is important to us that we offer the right support.  We recommend you submit your complaint in writing by email, letter or through our online submission form as this will ensure the details of your complaint are fully captured and from your own perspective.
